Prep Height, Collar Height & Profile Diameter: Choosing the Right Abutment Size

A sizing method for healing abutments and straight abutments, on any implant system.

Three dimensions decide how well an abutment performs at a given site. Titan stocks healing abutments and straight abutments across a broad range of each, so you can fit the tissue and the tooth instead of forcing the case to fit a one-size part. Used deliberately, that range saves chair time, cuts remakes, and produces a better-looking restoration.

The three dimensions

Get all three right and the component disappears into the case. Get any one wrong and you create rework.

Healing abutments: shaping tissue before you take an impression

The healing abutment's job is to pre-form the emergence profile during healing, so the soft tissue is already the right shape when you restore. Two of the three dimensions apply.

  • Match profile diameter to the future restoration, not just to the implant platform. A wide-diameter healing abutment under a molar develops a broad, supportive emergence; a narrow one under a lateral incisor preserves a slim, natural profile.
  • Match height to tissue depth. Too short and tissue grows over the healing abutment, forcing a second uncovering. Too tall and it is under-contoured and uncomfortable. Getting it right the first time avoids a return visit.
  • Treat it as free chair time. Tissue shaped correctly during healing means less soft-tissue management, and fewer surprises, at impression and delivery.

For the lab: a master cast that reflects a clean, correctly shaped emergence — so the crown contour is right the first time.

Straight abutments: collar height controls your margin

With a straight abutment, collar height determines where the crown margin sits relative to the gingiva.

  • Subgingival, equigingival, or supragingival — you choose with collar height. In the esthetic zone, a collar that places the margin slightly subgingival hides the metal-to-crown junction. In posterior or hygiene-critical cases, keeping the margin at or above tissue makes cement cleanup and maintenance far easier.
  • Keep the margin reachable. A correctly chosen collar height keeps the margin accessible for cement removal. Residual subgingival cement is well documented as a contributor to peri-implant inflammation, and margin placement is the main thing under your control.
  • Profile diameter still matters. Selecting the diameter that matches the tooth gives the crown a natural emergence and proper interproximal contacts, instead of a crown that is over-contoured or ledged.

Prep height: two ways to get there

Titan offers two approaches, and which one suits you is the main choice between our two straight abutment lines.

  • Adjust it chairside. The Straight Locking Implant Abutment Flat (SLIAF) ships at a standard prep height with horizontal grooves along the post. The grooves aid height measurement and adjustment, and increase cement retention while still allowing retrievability — so you can reduce to the opposing occlusion without losing grip on the crown.
  • Select it up front. The Simple Step Abutment is machined in three prep heights — 4.0mm, 5.5mm, and 7.0mm — with a hexed prep surface. Choosing the height at order time means less reduction at the chair and a hex that holds the crown oriented through fabrication and cementation.

A practical selection sequence

  1. Confirm the platform of the implant so the connection matches. Everything else follows from this.
  2. Measure tissue thickness by probing the cuff. This sets your collar height.
  3. Identify the tooth being replaced and its emergence width. This sets your profile diameter.
  4. Measure interocclusal space from gingival crest to opposing tooth. This sets your prep height.
  5. Order the specific combination rather than defaulting to a single stock size.

Tip: the best results come from taking a tissue model impression of the implant with a transfer coping. The lab can then select and modify the right abutment for the available space. The implant locator pin is useful here too — it lets you read angulation before you commit to a size.
