How to Choose the Right Abutment for a Biomet 3i Certain Internal Hex Implant

A step-by-step guide to selecting the correct Titan compatible abutment for T3 Certain, Osseotite Certain, Full Osseotite Certain, and Nanotite Certain implants. Covers platform identification by color code, abutment type selection, emergence profile, and collar height.

All T3 Certain, Osseotite Certain, Full Osseotite Certain, and Nanotite Certain implants share the same internal hex prosthetic connection. The color-coded platform is the only variable that determines which components you need. This is a different connection from the 3i external hex — components are not interchangeable.

STEP 1: IDENTIFY YOUR PLATFORM BY COLOR

The 3i Certain system uses four color-coded platforms. The platform color is documented in the surgical record or on the implant packaging.

Platform-switched Certain implants (4/3, 5/4, 6/5): The second number is the seating surface. A “5/4” Certain implant uses Yellow (5.0mm) packaging but seats on the Blue (4.1mm) platform — use 4.1mm components. Always confirm the seating surface from the surgical record, not the implant body diameter alone.

Step 2: Choose Your Abutment Type

Titan manufactures multiple abutment types for the 3i Certain internal hex:

  • Straight Locking Abutment: For cement-retained single-unit crowns. Internal hex engages the implant for anti-rotation. Multiple collar heights and emergence profiles.
  • Modifiable Abutment: A preparable abutment that can be shaped chairside or in the lab, similar to preparing a natural tooth. For cases requiring custom emergence or angulation correction.
  • Castable Abutment (Hex & Non-Hex): Burnout-compatible for custom cast frameworks. Hex version for engaging single-unit, non-hex for multi-unit or splinted restorations.
  • Temporary Abutment: For provisional restorations during healing. Textured surface for acrylic retention.
  • Ball Head Abutment: For overdenture retention using O-ring housings.

Step 3: Select Emergence Profile & Collar Height

For straight locking and modifiable abutments:

  • Emergence profile diameter: Match to the cervical diameter of the tooth being replaced. The abutment should support the interdental papillae without compressing them.
  • Collar height: Measure tissue depth from the implant platform to the gingival crest. Select a collar height that places the shoulder at or just below the gingival margin.

Titan offers more collar height and profile diameter options for the Certain system than the OEM catalog, including options for the purple (3.4mm) and green (6.0mm) platforms where OEM selection is limited.

Step 4: Confirm Seating Before Torquing

Before torquing the abutment screw, verify that the component is fully seated into the implant well. The internal hex should be fully engaged with no gap between the abutment base and the implant platform. Use Titan’s Implant Locator Pin to guide the abutment into the implant for parallel, accurate placement — especially useful in posterior sites with limited visibility.
